Vaccines Save Lives
We firmly believe that all children and young adults, with rare exception, should receive all of the recommended vaccines according to the schedule published by the Centers for Disease Control and the American Academy of Pediatrics.
Insurance
To be as accessible as possible to the families in our community, we accept most insurances. Please contact your insurance carrier to verify that Arvada Pediatric Associates is in their network and to verify your benefits and coverage.
Are you expecting a new addition to your family?
Congratulations! This is one of the most exciting times in your life. We strongly encourage parents-to-be to contact our office for a complimentary Meet the Doctor visit. This is a great way to get acquainted with our office and our providers, and we will answer any questions that you have about our practice.
When your baby is born, tell your obstetrician and the hospital staff that you have chosen Arvada Pediatric Associates as your child’s pediatrician. The hospital pediatric staff will care for your baby while in the hospital. Prior to discharge, please contact our office about scheduling your baby’s first visit.
Before Your First Visit
To expedite your first appointment, please download and print our new patient forms, fill them out ahead of time, and bring them with you to the appointment. If you are unable to complete the forms, please arrive a few minutes early to complete them.
Please provide the following information when you arrive for your visit:
- Insurance card(s)
- Photo ID
- Complete immunization records
- Medical records from your previous pediatrician
